The purpose of this gathering is to bring together leaders of faith and health to continue the dialogue on the intersection of faith and health to improve health outcomes for vulnerable communities. The forum will begin to put into practice recommendations from the 2012 forum series. Specifically, it will highlight concrete examples of successful faith and health community partnerships, demonstrate best practice models, and identify strategies for sustainability. In addition, it will include dialogue with academic deans and students on the realities of program integration, on strategies for developing formal partnerships, provide learning and networking opportunities, and it will encourage participant understanding of the importance of partnership building.
